- Birth Certificate
- Immunization Records
- Parent or Guardian Identification
- Proof of Residence
To confirm your local address, homeowners can provide a recent utility bill (water, gas, or electric) showing the address, bill date, and amount. Utility bill can not be more than 2 months old. Expired, past due or disconnected bills will not be accepted.
Renters could present a current signed lease agreement with the parent or guardian listed as the leaseholder or occupant. Please note, no subleasing or room rentals are permitted.
For unique living situations, an 'Establishing Proof of Residency' form must be completed by the parent or guardian and homeowner or leaseholder. Please contact the campus PEIMS clerk if you have a unique living situation.
Military families should provide their active Military ID.
